BRONX NEWS: NYCHA knew elevator that killed elderly resident was busted


Ninety minutes before an elderly tenant died in a lurching NYCHA elevator, the agency was warned that the lift was behaving erratically — but did nothing, a scathing new report released Tuesday found.


The city Department of Investigation uncovered failings, both specific and systemic, after 84-year-old Olegario Pabon died on Christmas Eve in a faulty NYCHA lift.


“There was a complete breakdown of communication at every level at NYCHA,” said DOI Commissioner Mark Peters, who recommended disciplinary action against 10 people, including the head of NYCHA’s elevator unit.


Mayor de Blasio vowed there will be hell to pay.


“The failure to repair the elevator immediately is completely unacceptable,” he said. “We will hold those responsible accountable.”


The investigation laid bare “significant flaws in NYCHA’s handling of elevator complaints, as well as NYCHA’s compliance with elevator safety laws.”


NYCHA TENANT, 81, DIES AFTER CRACKING SKULL IN ELEVATOR


A fail-safe device called a brake monitor that might have prevented Pabon’s death had been deliberately disabled, and DOI discovered brake monitor problems in 80 other elevators systemwide.


NYCHA also admitted to DOI that they “missed the boat” by not following a 2014 rule change requiring the annual inspection of elevator brake devices. The head of NYCHA’s elevator unit sat on the panel that drafted the rule.


Details of Pabon’s death were particularly unnerving.


At 2:38 p.m. on Dec. 24, a tenant called NYCHA’s command center to say an elevator in the Boston Road Plaza housing development for seniors in the Bronx was “very dangerous.” It was “going up and down by itself” with the door open, and when the door did close, “it slams real hard.”


The tenant left the same message with the development’s management office, but instead of dispatching a mechanic, NYCHA took no action at all.


The command center listed the elevator warning as a “low priority,” requiring inspection only within 48 hours. A development office secretary made little effort to get help, and then left her job at 4:30 p.m. sharp without contacting anyone.


She told DOI her “job is not to call the mechanic.”


Less than 90 minutes after the first warning calls, Pabon, walking with a cane, tried to enter the elevator, but the lift continued to rise with the door open. Pabon tripped and the door closed on his leg and hand. The door reopened and he fell back out into the hallway and fractured his skull.


Two days later he died of his injuries.


Three days before this incident, firefighters had to free a trapped passenger from the same elevator. And DOI found it took days before NYCHA leadership realized what happened.


What caused the elevator to go haywire remains unclear but DOI discovered the lift’s brake monitor — which shuts down elevators if the brake isn’t functioning — had been deliberately disabled, its wiring covered with electrical tape.


A NYCHA mechanic told DOI the brake monitors are “very sensitive” and “can cause issues” by unnecessarily shutting down elevators.


The non-functioning brake monitor at Boston Road was not unique.


After the accident, NYCHA inspected all 1,080 elevators with the same brake monitors. They found 80 weren’t functioning.


At the time of the accident, “NYCHA did not require mechanics to specifically examine and report on the condition of brake monitors,” the report states.


They also learned that the blueprints provided by the brake monitors’ manufacturer, Tricon, were misleading, allowing NYCHA mechanics to think the devices were functioning when they were not turned on.


Three weeks before the fatal accident at Boston Road, a remarkably similar incident occurred at the Morris Houses in the Bronx.


There tenant Jamal Brown was stepping into the lift when its floor kept rising with the door open. He tripped inside and the door closed on his foot and ankle.


For 20 minutes he struggled to free his foot. It took FDNY first-responders another 50 minutes to free him with the “jaws of life.”


He suffered a fractured foot.


DOI discovered missing bolts in the brake monitor needed to activate the system at the Morris Houses.


In response to DOI’s findings, NYCHA said Tuesday it fixed all the busted brake monitors and now requires mechanics to regularly check the devices.


Ritchie Torres (D-Bronx), chair of the City Council’s public housing committee, said he’ll hold a hearing on the problems uncovered by DOI.


“The fatality of Olegario Pabon and the elevator defect that led to it demonstrates a perfect storm of failure — from NYCHA staff to NYCHA leadership to the private manufacturer to the NYPD,” he said.

The Nike Air Presto is a perfect sneaker for the warmer months and expect Nike Sportswear to have a handful of releases. So far we have previewed the ‘White’ Nike Air Presto and around the same time, this ‘Racer Blue’ pair will be showing up at retailers.

Racer Blue has been showing up on more Nike Sportswear releases so it makes sense that the Air Presto gets the treatment. Featuring the shade of Blue through the uppers, other details include a White midsole, Black outsole and a transparent cage.

Although select retailers overseas appear to have released the Nike Air Presto ‘Racer Blue’, they won’t be widely available until April 2016. Although Air Max Day is over, Nike Sportswear will still have various Air Max models releasing that are ready for the Spring and Summer. One of those pairs to release soon is the Nike Air Max 90 Ultra BR ‘Volt’.

Anytime Volt is used on a pair of sneakers, it seems to catch the eye and in this case the Nike Air Max 90 Ultra BR has completely covered the shoes in the electrifying color. There is some Red that is used which is minimal and placed on the insole and the sole. The final touch is 3M reflective material used on the top of the tongue which is a nice touch.

Currently a release date for the ‘Volt’ Nike Air Max 90 Ultra BR has yet to be announced, but since the official images have leaked, they should be arriving to Nike Sportswear retailers and online in the coming weeks.

The Nike Kyrie 2 ‘Wolf Grey’ is the next signature shoe to release from Kyrie Irving and will be available at Nike retailers during April.

The Nike Kyrie 2 shown comes in a Wolf Grey, Dark Grey, Omega and Cool Grey color scheme. As you can see, shades of Grey flow through predominately while covering the uppers, mid-foot strap and even part of the midsole. Omega which is a shade of Blue hits the Kyrie branding while the Swoosh as an interesting cracked graphic. The last touch is White flowing through the outsole.

The Nike Dunk High ‘Be True to Your School’ is Returning



Nike Dunk Be True School White Royal

While March Madness is almost over and we are currently awaiting the Final Four bracket to take place, Nike Sportswear is bringing back the Nike Dunk High ‘Be True to Your School’ series.

So far, two colorways of the Nike Dunk High will return representing St. John’s and Kentucky, although both teams are not in the final four. The Kentucky edition comes in White and Varsity Blue while the St. John’s features White and University Red. Simple color blocking has made the ‘Be True’ series popular through the years.

The release date for the ‘Be True to Your School’ Nike Dunk High series has yet to be announced, but it appears that overseas retailers are now starting to receive shipments which would mean a release could be nearing. We will make sure to update you with additional images, release dates and if Nike decides to release some of the other classic models.
